Today's endodontic surgery bears little resemblance to that of two decades ago, and Torabinejad and Rubinstein show this procedure by procedure. After a brief review of the basic science of the tissues involved, the work moves on to diagnosis, planning and surgical technique, with magnification, ultrasonic tips, microinstruments, the new root-end filling materials and CBCT as its axes. It also deals with the maxillary sinus in periapical surgery, with soft and hard tissue healing and with adjunctive procedures — resorption, root amputation, hemisection, replantation, crown lengthening, grafting materials — and closes by evaluating outcomes in the light of current evidence.
