The atrophic arch tends to push the clinician towards bone grafting or sinus lift; Agliardi and Romeo argue for another route: angling the fixtures to move the support posteriorly, gain length and dispense with reconstructive surgery. The work starts from the science and the clinical evidence that underpin these protocols and goes on to patient assessment, diagnosis and planning before entering the technique. The procedures are developed separately for each arch and include All-on-4, trans-sinus and zygomatic implants and the authors' own V-II-V technique, illustrated with clinical cases from widely differing starting points.
