Apical periodontitis is, at bottom, an infectious disease: understanding what colonises the root canal system and how the host responds precedes any predictable treatment. Siqueira organises the volume into two blocks. The first covers microbiology and pathophysiology: biofilm communities and bacterial virulence, the interaction between pathogen and immune response, and the clinical manifestations of the disease. The second descends to technical detail: fluid dynamics applied to irrigation, supplementary disinfection of the canal system, emerging antimicrobials and the management of post-treatment apical periodontitis through non-surgical retreatment or periapical surgery.
